Mine has My Initials, My Surname and My Post Office Box Address (excluding Postal Area Code and State).
I leave it up to them to Notate (or Remember) My Christian Name when I Present the Card to them and to Look Up My Full Postal Address, when they are Writing a Snail Mail Letter to Me.
I am not interested in making everything too easy for them and I don’t want them wasting My Time In contacting Me for less than Significant Reasons.
No One who ever Truly Needed My Services has had too much difficulty contacting Me.
